This learning program seeks to elevate the status of pharmacists
as critical contributors to primary care.
Given the tremendous strides pharmacists have made in vaccination,
transitioning to chronic disease management (CDM) is a logical next step. Since “chronic conditions” are known risk
factors for severe manifestations of vaccine-preventable diseases, they should
be discussed with every patient at every visit!
